  "text": "Notwithstanding any other provision of this Chapter, the fees and costs established by the board in accordance with R.S. 37:1182(A) shall not be less than the following schedule:\n\nMinimum\n\n(1) Miscellaneous fees and costs\n\n(a) Photocopies of documents per page $ 0.50\n\n(b) Certification of document as true copy $ 5.00\n\n(c) Certification of document as office record $ 5.00\n\n(d) Certification of license $ 20.00\n\n(e) Official list of licensed pharmacists $ 150.00\n\n(f) Official list of certified technicians $ 150.00\n\n(g) Official list of pharmacy permits $ 150.00\n\n(h) Handling and mailing per page $ 1.00\n\n(i) Administrative hearing fee $ 250.00\n\n(j) Pharmacy intern registration $ 10.00\n\n(k) Law book $ 40.00\n\n(l) Certification of practical experience\n\nto another state $ 10.00\n\n(m) Official list of registered pharmacy interns $ 150.00\n\n(n) Official list of registered pharmacy\n\ntechnician candidates $ 150.00\n\n(2) Licenses, permits, certification, registrations,\n\nand examinations for pharmacists\n\n(a)(i) Renewal fee for license, per year $ 100.00\n\n(ii) Pharmacy education support fee, per year $ 100.00\n\n(b) Delinquent fee in addition to renewal fee,\n\nper year $ 50.00\n\n(c) Reinstatement of a license which has been\n\nsuspended, revoked, or has lapsed due to\n\nnonrenewal $ 200.00\n\n(d) Credential certification $ 50.00\n\n(e) Examination and licensing $ 300.00\n\n(f) Reciprocity fees $ 150.00\n\n(g) Certificates, duplicates $ 75.00\n\n(h) Certificates, silver $ 100.00\n\n(i) New issuance of certificates $ 75.00\n\n(3) Licenses, permits, certification, registration, and\n\nany other designations for pharmacy locations\n\n(a)(i) New pharmacy permit fee $ 150.00\n\n(ii) Pharmacy education support fee, per year $ 100.00\n\n(b) Pharmacy change of location $ 150.00\n\n(c) Pharmacy change of ownership $ 150.00\n\n(d) Pharmacy permit renewal fee, per year $ 125.00\n\n(e) Delinquent permit renewal fee, per year $ 62.50\n\n(f) Pharmacy CDS permit fee, per year $ 25.00\n\n(g) Delinquent pharmacy CDS permit fee, per year $ 12.50\n\n(h) Reinstatement of a permit which has\n\nbeen suspended, revoked, or which has\n\nlapsed due to nonrenewal $ 200.00\n\n(i) Automated medication system registration,\n\nper year $ 150.00\n\n(j) Emergency drug kits for long-term care\n\nfacilities, per year $ 25.00\n\n(4) Certification and examination for pharmacy\n\ntechnicians\n\n(a) Examination and certification $ 100.00\n\n(b) Renewal certification fee, per year $ 50.00\n\n(c) Certificate, duplicate $ 50.00\n\n(d) Reinstatement of a pharmacy technician\n\ncertificate which has been suspended,\n\nrevoked, or has lapsed by nonrenewal $ 200.00\n\n(e) Delinquent certificate renewal fee, per year $ 25.00\n\n(f) Pharmacy technician candidate registration $ 25.00\n\n(5) Per diem $ 75.00\n\n(6) Permits for pharmacy benefit managers\n\n(a) Application fee for initial permit $ 500.00\n\n(b) Permit renewal fee, per year $ 500.00\n\n(c) Delinquent permit renewal fee, per year $ 250.00",
