The board is authorized, by rule, in accordance with the Administrative Procedure Act, to establish fees and costs to be imposed for the purpose of implementing and enforcing the provisions of this Chapter. However, the board may, by a majority vote, reduce the amount of or waive the collection of any such fees. The fees established under the provisions of this Section shall be paid to the secretary-treasurer.\n\nB. Notwithstanding any other provision of this Chapter, the fees and costs established by the board shall be not less nor more than the range created by the following schedule:\n\n(1)Miscellaneous fees and costsMinimumMaximum\n\n(a) Photocopies of documents $ .50 $ 1.00\n\nper page\n\n(b) Certification of document $ 5.00 $ 10.00\n\nas true copy\n\n(c) Certification of document $ 5.00 $ 10.00\n\nas official record\n\n(d) Certification of good-standing $ 20.00 $ 40.00\n\nlicense\n\n(e) Official list of all licensed dentists $ 500.00 $ 1,000.00\n\n(f) Official list of all licensed dental $ 500.00 $ 1,000.00\n\nhygienists\n\n(g) Up to one-half of an official $ 250.00 $ 500.00\n\nlist of all licensed dentists or\n\nall licensed dental hygienists\n\n(h) Handling and mailing per page $ 1.00 $ 2.00\n\n(i) Bound copy of Dental Practice Act $ 35.00 $ 45.00\n\n(j) Unbound copy of Dental Practice Act $ 25.00 $ 35.00\n\n(k) Criminal history background check $ 50.00 $ 150.00\n\n(l) Retired volunteer dental license $ 50.00 $ 200.00\n\n(m) Expanded Duty Dental certificate\n\nconfirmation $ 100.00 $ 200.00\n\n(n) Review of proposed advertising $ 150.00 $ 300.00\n\n(o) Retired volunteer dental hygienist license $ 35.00\n\n(2) Licenses, permits, and examinations for dentists\n\n(a) (i) Examination and licensing\n\nof dental applicants $ 300.00 $ 1,200.00\n\n(ii) Make-up examination per portion $ 150.00 $ 250.00\n\n(iii) Licensing of dental applicants $ 300.00 $ 800.00\n\nwho have successfully completed\n\nan examination administered by\n\nanother jurisdiction which is\n\naccepted by the board\n\n(iv) Provisional dental license\n\nduring emergency $ 100.00 $ 300.00\n\n(b) Temporary dental license $ 100.00 $ 200.00\n\n(c) Issuance of a restricted license $ 200.00 $ 400.00\n\n(excluding advanced education\n\nstudents and dental residents)\n\n(d) Biennial renewal fee for dental $ 100.00 $ 600.00\n\nlicense\n\n(e) Annual or biannual renewal fee $ 100.00 $ 300.00\n\nfor restricted dental license\n\n(excluding advanced education\n\nstudents and dental residents)\n\n(f) Replacement of dental license, $ 50.00 $ 100.00\n\ncertificate, or temporary permit\n\n(g) Delinquency fee in addition to $ 250.00 $ 400.00\n\nrenewal fee for any dental license\n\n(h) Reinstatement of a license which $ 500.00 $ 800.00\n\nhas been suspended, revoked, or\n\nwhich has lapsed by nonrenewal\n\n(i) Restricted dental license, advanced\n\neducation students and dental residents:\n\n(i) For period July 1 -
